Targeted Memory Program Results – Case Study 06010PAG

“Assistance in Assisted Living”

Client Review:
This female client has low vision and behavior issues. She was targeted by the management of the Assisted Living Facility to move onto the secure memory care unit due to her constant calling out, putting on the call bell, repeating questions and lack of stimulation due to sleeping. The Homewatch Franchise offered to conduct a demonstration of the Pathways to Memory™ program at no cost to the client or the facility to show how well the program works. The facility and the client’s responsible party agreed.

The Journey Begins:

The First Session:
The client seemed very anxious during her initial assessment. She asked many times why the Partner was asking so many questions and what was she supposed to do next. The Partner took her time with the client and patiently answered her repeated questions. The Partner felt it was clear that part of the client’s anxiety was related to her low vision and her feelings of disorientation.

By The 3rd Week:
The Partner reports that she and the client have begun to form a friendship and a bond. They continue to work on orientation within the facility – The Partner showing the client how to identify where she is in the building and how to locate the dining room, the nurses station, the activity room – and they are working hard on cognitive enhancements, attention span and memory concentration. The Assisted Living Facility staff reports that the client is sleeping better at night and is more awake and alert during the day. The staff also reports that the client is already showing less anxiety than before she started the program.

By the 10th Session
The client is visibly less anxious. She no longer calls out for staff and she is using her call bell less frequently. The staff reports “her old spunk is back” and she seems more content and happy. The Management staff of the Assisted Living facility have removed the client from the list of persons recommended for the Memory Care Unit as she no longer qualifies.

Results and Outcomes:

  • Initial MMSE score: 21
  • Concluding MMSE score: 25
  • Increase of 19% in cognitive ability
  • Initial 7 minute screen score: 25
  • Concluding 7 minute screen score: 35
  • Increase of 40%

The Journey Continues:
The changes were so positive and noticeable to everyone in the facility as well as to the client’s responsible party that they immediately signed the client up to a full 12 week program.

MMSE – 25 Increased by another point – 27% increase overall
7 minute screen 58 Increased by 24 points - 57% increase overall
