We understand the importance of talking to a live person during those delicate and difficult times.  Therefore, our practice is that a trained Homewatch CareGivers staff member answers all calls 24 hours per day/7days per week.  When a return call or email is necessary or if there are technical issues, we will communicate with you within 1 hour during normal business hours and within 2 hours outside mormal business hours.

Doing it all Remotely

We specialize in Nurse Care Coordination of clients with long distance families.  Care Coordination includes helping with a seamless discharge from a facility, assessing and ordering needed durable medical equipment, home safety and fall risk evaluation to assure clients are safe at home, setup and monitoring medication management systems to decrease the risk of under or over medicating, assessing the need for remote monitoring to assure safety, monitoring nutritional needs and assisting with ordering prepared meals and frequent communication and updates with family.  Therefore, we fullfill our mission to "Provide Peace of Mind".

Client Evaluation

Our evaluation process begins with a Registered Nurse assessing your loved ones needs during a complimentary evaluation at home or at the facility where the client is staying, so that together we can determine the best plan of care for keeping them home safely.  We can schedule an evaluation within 2-4 hours of the request and the process takes a 11/2 - 2 hours.  We obtain the needed information, develope a 'Customized Care Plan' and decide on a schedule that meets the clients physical, emotional and safety needs. 


 

Completing the Paperwork

Our nurse can complete and obtain signatures for all necessary paperwork to begin care during the evaluation in the home or at the facility. We can accept signed faxed or signed emailed copies, when a signature is required from an individual other than the client, to begin care.  Nevertheless, we are require to have the original signed documents on file within a week of statrting care.

Starting Care within 4 Hours of First Contact

We take pride in being able to start providing quality care to our clients within 4 hours of completing the evaluation.  We know you can't always plan life's events in advance.  However, we are confident that we can provide you and your family exceptional home care while staying safe at home.
