Alzheimers and Dementia Care Program

Pathways to Memory is a coping solution for the entire family, who are now living with dementia. Our mission is to promote quality of life and foster independence in a safe, failure-free environment.

Specialized Dementia Care 

Personal care for those living with memory impairment. Providing activities of daily living tailored to the needs of those with memory impairment, including: 

• Bathing 

• Dressing 

• Personal Grooming 

• Housekeeping 

• Transportation 

• Healthy Meal Preparation 

- Quality companionship is just as important; our Specialized Dementia Care provides appropriate engagement activities based on your loved one's social history, hobbies, and personal preferences. 

-Care is provided by intensively trained and tested caregivers with 7 hours of dementia-specific class work, plus supplemental hands-on training. 

Focused Memory Training 

Personal coaching to exercise the brain and foster independence

Our Focused Memory Training offers one-on-one coaching and instruction: 

• The 2 hour training sesions occur 3 times a week and last for 12 weeks. 

• The program plan is designed based on an assesment of the client's ability in the 6 cognitive areas of human memory. 

• Activities and goals are designed to meet the unique challenges of each individual client.The program cultivates self-sufficiency, competency, and self-worth in an encouraging, "failure-free" environment - while promoting a sense of purpose and memory skills needed for life's daily activities.

The consistency of the training enables the formation of a trusting relationship between the client and the Pathways to Memory caregiver, and the entire family.
