Homewatch CareGivers is committed to being one of the top home care agencies in Santa Rosa through personalized, expert care. The need for care is not a Monday through Friday, 9-5 situation. People need care at all hours of the day or night. Homewatch CareGivers of Santa Rosa has a typical course of action for bringing on new clients—and we acknowledge that in times of emergency, that process might need expedition. For most clients, the process starts with a phone call—a first impression on both sides to discover what needs are met and what the client—or client’s family member—might have as expectations.

Immediate Care, No Matter Where You Are

We can start taking care of your family member immediately; even if you are not available in person to sign paperwork on behalf of your loved one. This includes a seamless discharge from a facility. You can complete any paperwork via email or fax, if your loved one cannot do so.

Evaluating Our Care and Your Needs

We come to you and create a custom plan

Scheduling the Evaluation

We can come to your loved one's home or meet at the facility where your loved one is currently staying to conduct a comprehensive evaluation on your loved one's needs. In most cases, we can conduct the evaluation the same day you reach out to us.

Creating a Care Plan

After the phone call, we schedule an evaluation interview, where we want to learn as much as possible about our clients, not only their medical condition and care needs, but their likes and dislikes, their personalities. This assists us in finding a perfect match between the caregiver and client. At the end of the evaluation, we discuss the desired schedule. Once we have the signed agreement, we put together a plan of care and a schedule. On the first day of service with a new caregiver, we are there to introduce the client and caregiver to each other and show the caregiver around the home. After the first few shifts, we call the client to check in and see if we need to adjust the plan of care or the schedule in any way.

We can make available to you weekly progress notes about your loved one's weekly activities.

Keeping Everyone in the Loop

Family communications can be one the most important aspects of our job as a home care agency. Staying on top of changing social schedules, running necessary errands, sharing notes from doctor’s visits, and communicating about your loved one’s general well-being are all important components to family communication.

How are We Doing?

We will ask you from time to time for feedback of how we are doing. It is the only way we know to make certain we are continuing to deliver the quality care you expect from us. We stay on top of things through informal communications and Quality Assurance visits.
