What is special about you? What is your personal history and what should be avoided? How can we assist you to maintain your independence? Some clients require guidance with meal preparation while others live in a facility, some require an escort to appointments while others need supervision to perform their activities of daily living. The Plan of Care gives us the information needed to match the right level of caregiver and provides us with essential medical information to help us be partners in your care. This information is entered into our internet-based program and can be updated at any time.
