Chronic conditions are plaguing our health care system, and
in March of 2010, the Affordable Care Act was put in to place to help avoid many
of the issues associated with conditions such as heart attacks, heart failure, and
pneumonia. Our hospital systems are being fined for readmissions when people
have these chronic conditions. In Highland Park, as part of care for chronic
conditions, we’re working with hospitals and rehabilitation facilities to make
sure our clients don’t get readmitted to the hospital.
One of our clients, “Jim,” had a stroke and this
dramatically reduced his physical abilities. We worked with his family, his
doctor, and the medical community to come up with a plan to get him home
safely. We brought our caregiver into the rehab facility for training on how to
do physical exercises, how to go through physical therapy programs, and so Jim
could gain a comfort level with the caregiver. On discharge day, we brought Jim
home, helped him settle in, picked up his medications, and began implementing
the exercise program as we worked closely with the skilled nursing team. His
progress was incredible and today he is 100% recovered and back to doing the
things he loves.
