You can reach us by phone 24 hours a day, 7 days a week because your call is important to us. If we happen to miss your call, we will return your message within 1 hour.
Expect a reply within 1 hour between 8am and 6pm, or by 9am the following day if after 6pm evenings and weekends.
We can come to your loved one's home or meet at the facility where your loved one is currently staying to conduct a comprehensive evaluation of your loved ones needs. In most cases we can conduct the evaluation the same day you reach out to us.
We create our person-directed Care Plan during our initial evaluation with your loved one and the other partners on the care team. We take into account the client’s needs, abilities, preferences, and contributions of other care partners. What we learn is used to make sure that our caregiver’s skills and personality blend well with your loved one’s lifestyle and preferences.
It doesn’t stop there. As we get to know each client more deeply, and as their needs change, we continue to update the Care Plan to ensure that we and our caregivers continue as good partners in care. We are actively looking for ways to care that create opportunities for your loved one to live a full and purposeful life according to their preference and abilities.
Our supervisors stay in touch with clients and families at a frequency that fits your preference and your loved one’s needs. This may be daily or more during times of transition and changing health or it may be weekly or less when that is the established routine. We encourage your visits and calls while our caregivers are on duty. We also provide quarterly summaries from our Daily Client Status charting so we can review with you changes in mobility, emotional status, comfort level, and cognitive functioning.
We want your feedback! How are we doing and what else can we do? Our nurses stop by on Quality Assurance visits monthly. We use Home Care Pulse surveys to measure both our clients’ and our caregivers’ satisfaction and to adjust what we are doing to constantly improve both. We are in care partnership with you to provide the best quality of life possible for our clients and families.
