Care starts with a phone call from a potential
client or a healthcare professional. During this phone call we collect some
general information about the client and his/her physical and mental condition.
If we are confident that we are able to provide care, we will schedule one of
our nurses to do an evaluation. You can reach us by phone 24 hours a day, 7 days a week. Our office is open between 8 am and 5 pm Monday - Friday. After business hours and on weekends please call 919-960-8142919-960-8142 to speak with an on-call emergency staffing coordinator.
We monitor our emails 7 days a week. Expect a reply within 1 hour between 8am and 6pm, or by 9am the following day if after 6pm evenings and weekends.
We can start taking care of your family member immediately after the initial assessment is complete; even if you are not available in person to sign paperwork on behalf of your loved one. This includes a seamless discharge from a hospital or a transition from a facility back home. You can complete any paperwork via mail, email or fax, if your loved one cannot do so.
Evaluations are an essential part of our
admission process during which we learn more about the client’s condition, home
safety, family dynamics, and other important information, which helps us to
create a customized plan of care, perfectly suited to meet the each client’s
unique needs. Then, through close collaboration of the nurse, scheduling
coordinator, and client care coordinator, we select the most suitable caregiver
for this particular client. Once the right caregiver is identified, we begin
services within a day or two, but in an emergency situation, we can begin
services the day of the initial phone call. Our admissions nurse can come to your loved one’s home or meet at the facility where your loved one is currently staying to conduct a comprehensive evaluation of your loved one's needs. In most cases we can conduct the evaluation the same day you reach out to us.
We create a Plan of Care by first learning about your loved one’s family history, preferred social activities, daily routines, medication regimen, medical history and a review of other healthcare providers' recommendations. All this information is used to begin our caregiver matching process to make sure your loved one’s lifestyle and care needs are met by the most qualified and skilled Homewatch caregiver. We also conduct a thorough fall-risk and safety assessment of your loved one’s home to make sure the care is provided in the safe environment.
Care notes regarding your loved one's daily activities are documented by our caregivers and are available in special journals kept at the client's home. A new way of documenting client's activities using our Lawton tool will be implemented soon.
Clear and timely communication with the families is one the most important responsibilities that our agency is charged with. Staying on top of changing social schedules, running necessary errands, sharing notes from doctor’s visits, and communicating about your loved one’s general well-being are all important components of keeping the family in the loop.
Shortly after services begin, our nurse
supervisors follow up with the client and the family on a regular basis to make
sure they are happy with our services. From time to time we will ask you for a feedback on how we are doing, either by phone, mail or email. These surveys are performed by an independent customer satisfaction company Home Care Pulse. These are the only ways we know to stay on top of things and to make certain we continue to deliver the highest quality of care you expect from us.
