We can talk with you about care needs anytime, even on weekends.  Discharges from hospitals in North San Antonio can happen anytime and we’re ready to be there for you.  If we happen to miss your call, we will return a message left within one hour anytime, day or night.

We closely watch our email, san-antonio@homewatchcaregivers.com during business hours and will respond within one hour between normal business hours of 7:30am and 4:30pm, or next business day during evenings, weekends and holidays.  Be sure to call us if contact is urgent.

Immediate Care, No Matter Where You Are

We can make arrangements and begin care anytime even if you are not available to meet in person to finalize paperwork on behalf of your loved one.  We understand that if the situation calls for it, we will work to support a seamless discharge from any facility in North San Antonio.  You can complete paperwork through email or fax if your loved one cannot do so.

Scheduling the Evaluation

With no obligation, we would like to meet at your loved one’s home or temporary residence to conduct a comprehensive evaluation of your loved one’s needs.  If it works for you, we can normally meet the same day you call us if needed.

Creating a Care Plan

As the premier home care agency in North San Antonio, we want to first know your loved one’s family history, social activities, likes and dislikes, interests and desires, medication regimen and of course medical history.  We also want to know what you’re looking for in a caregiver.  We use this critical information to begin our caregiving matching process making sure our caregiver fits your your loved one’s lifestyle.  We will also make a thorough fall-risk assessment of your loved one’s home because falls are the leading cause of injuries among seniors.


Partnering in Care

We provide updates on your schedule and will listen to feedback.

Care Progress Reviews

You determine the frequency and we will provide updates and progress notes about your loved one’s activities and well being.
