JOB DESCRIPTION

Job Description: Caregivers are either certified nursing assistants (CNAs) or Personal Care Providers (PCPs) and work under the supervision of a nurse to help clients with daily living tasks. Nature of job: Caregivers provide paraprofessional services to the agency’s home care clients, as written in the care plan and in compliance with the client’s Service Agreement. Working closely with clients, Caregivers are responsible for basic care services such as bathing, grooming and feeding clients, assisting nurses with medical equipment, and checking client vital signs. Caregivers give clients important social and emotional support and also provide vital information on client conditions to nurses. Each caregiver must: • Provide care to clients as specified on the individualized care plan and assignment sheets • Provide a safe home environment • Provide assistance with the activities of daily living, physical comfort, and emotional and social support under the supervision of VP of clinical services (a Registered Nurse) or designated nursing supervisor • Caregivers must not not administer medication or perform any procedures requiring the knowledge or skills of a Registered Nurse or licensed therapist • Caregivers are never expected or permitted to perform any duties or tasks for which they are not trained or qualified In some cases, clients may need transportation to appointments or assistance running errands. Caregivers must be observant enough to recognize changes in their clients, and be capable of alerting the appropriate personnel when necessary. Reporting: • Reports to the VP of Clinical Services for the Grand Strand Homewatch CareGivers or designated nursing supervisor Major Responsibilities and Duties: • To continuously provide quality of care to all clients, treat them with dignity and respect, and know, follow all Grand Strand Homewatch Caregiver policies and procedures • To assist clients and families in regaining abilities and to maximize their strength in order to create an atmosphere of independence. • Responsible for assisting clients with their personal care needs as per their individualized care plan in accordance with the certification qualifications • Responsible for assisting clients with activities of daily living in a safe and beneficial manner, reporting any concerns to the VP of Clinical Services or designated supervisor • Must know and follow the existing lines or communication and authority • Follow all safety, security, infection control, Universal Precautions and Hazardous Material Policies and Procedures • Maintain confidentiality of all clients • Required to work harmoniously with the entire Grand Strand Homewatch Caregiver team • Must know and respond to all emergency situations involving the safety of clients, other employees and facilities Qualifications: • Able to work an average of 20-40 hours per week • Able to continuously walk, bend, climb, stoop, and stand an average of 5 hours per day • Able transfer, lift, move, turn and assist client to or from bed, toilet, commode, tub, shower or chair • Able to lift a minimum of 30 pounds • Able to use tools necessary for job including but not limited to: thermometers, blood pressure equipment, and transfer/mobility aids (such as walker, cane, wheelchair, etc) • Able to communicate clearly and precisely • Able to notice changes in clients’ condition (breathing, color, skin breakdown etc.), detect odors (spoiled food, soiled linens, smoke etc.) and hear emergency signals and act accordingly, notifying the appropriate personnel

REQUIREMENTS

COMPETENCIES REQUIRED • Must be a Certified Nurse Assistant or currently enrolled in a certified nurse assistant course or have met exempt qualifications set forth by South Carolina Department of Public Health • Must be registered on State Nurse Aide Registry in State where completed certification Nurse Assistant course • Must maintain CNA or equivalent certification • Must possess a valid driver’s license and to have current automobile insurance. General Knowledge, Skills, and Abilities required: • Experience as a caregiver in a home care setting or skilled facility • Must meet hiring criteria as defined by Homewatch CareGivers minimum standards • Must be able to function with minimal supervision, accepting personal responsibility for maintaining a professional relationship with the client • Must accept responsibility for maintaining skills and learning on an ongoing basis, as well as adhering to agency policies • Satisfactory background screening results • Attend and complete all annual training requirements, • Attend all required meetings & in-service education sessions (minimum 12 hrs each year) • Be able to greet and direct family members in a friendly manner • Maybe assigned other responsibilities and needed
