The ideal Care Coordinator is a qualified person appointed by the Governing Body/Owner to coordinate the care services of the agency, its employees and clients.

    The appointed Care Coordinator for the agency must meet the following criteria:

Experience and Training:

  • Must have great time-management skills and the ability to effectively multi-task

  • Must be organized, detail oriented and meet deadlines

  • Strong interpersonal skills and the ability to work effectively, and communicate information with management, care managers, clients and caregivers over the telephone, email and in person in a professional manner

  • Excellent writing and communication skills (documenting, email, phone, face-to-face)

  • Staff scheduling experience a plus

  • Experience as a caregiver in a home care setting or skilled facility
  • Must pass national and multi-jurisdictional background checks, Department of Motor Vehicle checks

  • Must be able to meet quality standards of care during shifts and follow standard procedures for engaging with the rest of the care team

  • Must possess a valid driver’s license and automobile insurance

Knowledge, Skills, and Abilities Required:

  • Creating and maintaining client and caregiver schedules and details via a software database
  • Professionally handling daily inbound and outbound communications with clients, care managers, family and caregivers (email, text, phone)
  • The ability to gather data, compile information and implement needed information, and to prepare care plans, and reports to share with the team, including family and clients
  • Conducting home visits and ensuring client needs are being met, and resolving issues as needed
  • Must be able to work as a caregiver, cover shifts whenever necessary due to caregiver call-outs, and other scheduling conflicts
