In-Home Personal & Respite Care for PHC Members

At Homewatch CareGivers of Santa Rosa, we are proud to partner with the Partnership HealthPlan of California (PHC) to offer personal care and respite services to eligible Medi-Cal members under the CalAIM Community Supports program.

What Is CalAIM?

California Advancing and Innovating Medi-Cal (CalAIM) is a state initiative designed to address not only medical needs, but also the social factors that impact health. Through CalAIM’s Community Supports, Medi-Cal managed care members can access critical non-medical services—like in-home care—that help prevent unnecessary hospitalizations and long-term facility stays.

Who Qualifies?

You may be eligible if you’re a PHC Medi-Cal member and:

• You’re applying for In-Home Supportive Services (IHSS)
• You need more IHSS hours
• You don’t have a caregiver available

Support with bathing, dressing, hygiene, and mobility.

Respite Care

Temporary relief for family caregivers when they need a break.
