When you are ready to begin services with Homewatch CareGivers of Haddon Township/Westmont, we will need to have one of our nurse’s write a plan of care for you. Our professional care team will come and meet with you and learn as much as we can about you, what makes you tick, what do you like and what do you dislike, what do you need us to assist you with in order for you to remain safely and independently in your home.  Then, our nurse will come over and perform her in-depth evaluation. From all the information that we gather about you and your care needs, the nurse will formulate your individualized plan of care.  The plan of care is basically the written instructions for the caregivers as they work with each client.  She will then instruct the caregiver(s) who will be helping you on your wants and needs, reviewing the care plan with the caregivers and making sure that their skills match the needs written out in each specific plan of care.  Once the plan of care is done, services can begin!
