Homewatch CareGivers of Yorba Linda, begins each caregiving service with a complimentary
assessment of our clients’ needs and wants to create a personal
plan of care. We address our client’s abilities and problems. We
then provide a “road map” to guide all our caregivers who
are involved in our clients’ care.
“What can we do to provide optimal quality of life, comfort and dignity
for this person?” From companionship services that help to alleviate
loneliness, to hands-on care with personal hygiene, our caregivers not
only follow the care plans, but also report back to our office to ensure
that we are constantly updated with our clients’ condition. A care
plan is always fluid, and changeable, as our clients’ status changes.
