Sometimes care needs to start with very short notice and we do our best in Edina to respond quickly and help however we can to start care today. One of our families was in a situation where the client was released from a rehab with only one day’s notice and the wife was struggling to handle all the details and get him home on her own! So we met her at the rehab facility, helped pack up all of his belongings and assisted getting him from his wheel chair into the car. Then we followed the family home, again transferring him from car to wheelchair and into the house. Once the client was settled, we set about making dinner for him and his wife. Our assistance made the transition home safer for the client and easier for his wife – it would have been very hard for her to manage this on her own.
We answer the phone 24 hours a day, 7 days a week because your call is important to us. If we happen to miss your call, please leave a message with your phone number and we'll contact you within one hour during normal business hours and within three hours during the evening and on weekends.
We monitor our emails seven days a week. You can expect a reply within an hour if you e-mail us during normal business hours (8am - 5pm - Monday through Friday), or by 9am the following business day if after 5pm evenings or on the weekend.
We can usually start taking care of your family member immediately, even if you aren't available in person to sign paperwork on their behalf. This includes a seamless discharge from a facility. Any paperwork can be completed by e-mail or fax if your loved one can't do so on their own.
We can come to your loved one's home to conduct a comprehensive evaluation of your loved ones needs. If they are currently in a facility, we can do an initial meeting in the facility with an additional follow-up once your loved one is home. In most cases we can conduct the evaluation the same day you reach out to us.
We begin creating a care plan by first learning about your loved one's family, medical and social history. We ask about personal preferences and enjoyed activities in order to begin our caregiver matching process. We also conduct a vulnerable adult assessment and a thorough fall-risk assessment of your loved one's home.
We track your loved one's daily and weekly activities using our progress notes and can provide copies to you by mail or e-mail.
We promote open and regular communication between our agency and our client families - we consider you our partners in care. Staying on top of appointments and doctor's visits, running errands or grocery shopping and communicating about your loved one's general well-being are all ways in which we partner with our families to provide the best quality care.
From time to time we'll ask you for feedback on how we're doing. Regular in-home Quality Assurance visits by our RN/Care Manager and Home Care Pulse phone surveys are just two of the ways we check in with you to make sure we're delivering the highest quality care.
