What Is Transitional Care?

Sometimes individuals need to transition from home to community living. Until the move can be made, transitional home care services, or bridge care, can keep potential residents safe in their current homes. 

At Homewatch CareGivers, we will customize a transitional plan of care to provide continuity as the person moves from one home to the next. Our process starts with a safety and fall consultation to identify any risks or concerns. We will also assist after the move is complete, as your loved one becomes acclimated to their new group home setting.

What Does Senior Bridge Care Look Like?

A loved one may be ready for a community care or nursing home setting for any number of reasons, however, placements are not always immediate. 

In-home transitional care is designed to provide the individual with what they need to stay safe and cared for until the move can be made. 

It consists of:

  • A fall risk assessment
  • Medication reminders
  • Healthy meal planning and preparation
  • Transportation services to appointments and social outings
  • Assistance with shopping for groceries and other items
  • Light housekeeping
  • Laundry and linen changes
  • Customized companionship

Gradually introducing a person to their new community often makes the transition into a full-time resident more comfortable. Through a series of visits, our caregivers are dedicated to helping the new resident develop engaging relationships with their new neighbors.

Bridge care to community living involves:

  • Establishing relationships and developing trust with the community and staff.
  • Establishing friendships with existing residents.
  • Engaging in activities, meals, and outings.
  • Becoming familiar and comfortable with life in the community.
