Ann Arbor Plan of Care
Each plan of care is developed in most cases by our Director of Client
Services, Angelique Fluellen in Ann Arbor. Angelique is an MSW and has
an extensive background in patient care and counseling with a personal
and professional background.
The plan of care is initially discussed in the complementary evaluation.
However, because our home health agency recognizes that lives and conditions
change, we make a point of engaging the entire team to influence the plan
of care throughout the entire relationship. These changes and attributes
can be suggested by the client, client’s family, caregivers, community
partners that share in the care of a patient. Of course, our Director
of Clients services confirms information and changes and updates the care plan.
