Loudoun, VA Plan of Care
Every one of our clients has a plan of care written and approved by a Registered
Nurse. Your loved one’s plan of care will take into account his
or her specific needs and will be written in collaboration with the entire
family. The plan of care provides specific instructions for the caregiver
and helps to assure that everyone understands the specific care your loved
one needs.
The plan of care is a living document that can change as the client’s
needs change. It addresses not only the client’s physical needs,
but also the client’s likes and favorite pastimes. Once the plan
is agreed, the Homewatch CareGivers of Sterling team matches the right caregiver to
the client.
Each of our clients has a Client Notebook kept at home that contains the
plan of care and detailed records of the care received. The care we provide
and the client’s condition are well documented in the client book,
providing a great resource for the family and the client’s physician.
